The Australian Transport Safety Bureau (ATSB) released its final investigation report (AO-2026-004) on August 17, 2026, revealing that a maintenance work light was left inside the left wing of a Qantas Airbus A380 during scheduled work at Sydney Airport (SYD). The aircraft subsequently operated two long-haul international passenger flights before the tool was discovered and recovered.

The aircraft involved, an Airbus A380-842 registered VH-OQK, arrived at Sydney Airport from Los Angeles International Airport (LAX) on January 7, 2026. Maintenance engineers performed work on its air conditioning system, accessing an internal compartment of the left wing to replace an outlet temperature sensor on an Air Generation Unit. A portable work light was brought inside to illuminate the workspace. Following the maintenance, engineers performed foreign object clearance inspections but failed to detect the light left inside the wing. Subsequent tool return procedures and end-of-shift checks also failed to identify the missing tool.

Despite the unreturned tool, a licensed aircraft maintenance engineer issued a certificate of release to service, allowing the aircraft to return to operation. The A380 departed Sydney later on January 7 bound for Dallas/Fort Worth International Airport (DFW), arriving on January 8. External maintenance personnel at Dallas/Fort Worth performed further work on the aircraft, but the maintenance lamp again went undetected. The aircraft then operated return flight QF8 back to Sydney, landing at 06:55 on January 9.

The missing light was identified later that morning when a Qantas tool crib operator reviewed equipment records and noticed that a work light issued for VH-OQK two days earlier had not been returned. Maintenance personnel initiated a search and located the lamp near the Air Generation Unit heat exchanger inside the left wing. The ATSB confirmed that the aircraft sustained no structural damage, and no system disruptions or injuries occurred during the flight sectors.
